Building Production RAG Systems with Hybrid Vector Search and Zero Hallucination

The Challenge of Enterprise Hallucination

Deploying Large Language Models in regulated enterprise settings requires zero tolerance for hallucinations. Typical naive retrieval-augmented generation (RAG) pipelines often suffer from vector search blindness, retrieval mismatch, and poor document chunking. Here is our battle-tested production blueprint.

Hybrid Dense & Sparse Search

Relying exclusively on dense vector embeddings misses critical exact-match keywords such as contract clause numbers, compliance codes, and specific SKU identifiers. By combining dense embeddings (OpenAI text-embedding-3-large) with sparse lexical algorithms (BM25) via a reciprocal rank fusion (RRF) reranker, retrieval precision jumps by over 38%.

Enforcing Role-Based Access Control (RBAC) at Retrieval Level

Vector databases must never bypass enterprise access policies. We implement pre-retrieval metadata filtering directly inside Pinecone and Milvus queries, guaranteeing that LLM context windows only receive text chunks authorized for the authenticated user session.

Deterministic Citation Extraction

To eliminate speculative output, every answer is synthesized with mandatory footnote references linking directly to verified passage IDs. If retrieved context confidence falls below 85%, the system deterministically falls back to safe deflection rather than speculative guessing.
